hereby assume so the rule of participating in True Gospel Christian Center Church / True Gospel Recovery Center activities and or events (3 months)
To the best of my knowledge I am physically fit and have not been advised to not participate by a medical health professional. I am not aware of any health-related reasons or problems that preclude my participation in this training or activity.
I hereby acknowledge that this Accident Waiver and Release of Liability contained herein will be used by True Gospel Christian Center Church and True Gospel Recovery Center in the event of an accident or injury to me from my own negligence or that of a third party.
I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, all claims and liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, theft of property, violations of any federal, state, local statute, and or regulations resulting in fines, penalties, and incarceration and or actions resulting in arrest, and all actions of any kind which may hereafter occur to me as of these activities, event and or in the Recovery Center and to and and during my traveling to and from these activities/events. THE FOLLOWING ENTITIES AND OR PERSONS: True Gospel Christian Center Church, True Gospel Recovery Center, all of theirs, the directors, officers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;
I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this agreement.
ACCIDENT WAIVER AND RELEASE OF LIABILITY from any and all liabilities or claims made as a result of participation in any activity and or event, whether caused by negligence or otherwise.
I acknowledge that True Gospel Christian Center Church/ True Gospel Recovery Center of Texas and their directors, officers, employees, volunteers, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity (including a specific event or activity volunteer) I have volunteered to participate.
The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. All references to all activities and or activities and the Recovery Center itself, including but not limited to the activities detailed herein.
I, the undersigned do freely volunteer to allow True Gospel Christian Center Church and/ or True Gospel Recovery
Recovery Center, to record via audio, video, and or taking pictures of me. I understand and agree that these materials may be edited and included in TV and radio broadcasts, the internet, and printed.
additional law and or other forms of communications, formatted and created for use by True Gospel Christian Center Church/ True Gospel Recovery Center including websites. I also understand that any information I receive from these Ministries is for my personal guidance and ONLY. The Ministries are not operating as qualified counselors or medical professionals and are not qualified to give advice outside of spiritual guidance and are not liable for any advice given, by anyone, outside of spiritual guidance.
The rules of True Gospel Recovery Center are posted in the Sanctuary. I hereby assume the responsibility of reading and familiarizing myself with them. I further understand that from this date forward it is my responsibility to follow the rules that are posted or non-posted (verbal). The rules are subject to change with or without prior notice.
This agreement also means that I am here for 2 Days if this is my first time here, from the date this PARTICIPATION AGREEMENT is signed on. I am living in the Recovery Center on TRIAL BASIS and could be asked to leave if and when the DIRECTOR or other Ministry representative deems it to be necessary. I am not a good fit for PARTICIPATION AGREEMENT. I understand I can leave at any time. I also acknowledge if I violate the rules of True Gospel Recovery Center and am asked to leave at that point I am considered to be trespassing and the POLICE can and will be summoned to escort me off the premises if I refused to leave of my own free will.
I CERTIFY THAT I HAVE READ THIS DOCUMENT IN ITS ENTIRETY AND I FULLY UNDERSTAND ITS CONTENT AND ASSUME THE RESPONSIBILITIES THEREIN. I AM AWARE THAT THIS AMONG OTHER THINGS IS A RELEASE OF LIABILITY AND A CONTRACT AND ISIGN OF MY OWN FREE WILL.
